Student Screening Instructions & Information

Dear Parents and Guardians,
The Commack Public Schools are reopening on Monday, September 14, 2020. On this day, our elementary school students will be returning to school five-days per week and our secondary school students, Commack Middle School and Commack High School students, return to an alternate day (A/B) schedule. 

New York State Department of Health and New York State Education Department guidance requires that a screening questionnaire be completed for each student. The purpose of this letter is to provide families with information regarding this screening questionnaire. 

Completion of this questionnaire by the parent/guardian of each student is required every morning (daily) that school is in session. The questionnaire is to be completed before your child leaves for school. If the screening questionnaire is not completed, and your child arrives at school, please know that your child will be isolated, and you will be contacted to complete the screening questionnaire. Depending on how the questionnaire is completed, your child may return to class or you will be asked to pick up your child.

You will receive an invitation to complete this screening, daily, via the email address listed in the Parent Portal as Emergency Contact 1. You can also opt-in to receive text message invitations to complete the screening questionnaire. In case you do not receive an invitation, there is a solution that will allow you to complete the screening questionnaire on a temporary basis while being corrected. More details can be found in the attached directions and FAQ.

Screening Questionnaire: 
Below are the screening questionnaire statements that parents/guardians will attest to for each student. The questionnaire will ask additional questions as well. For example, you will be asked if your child will be attending in-person instruction, remote instruction, or will be absent from school. 
 
Completion of the daily screening questionnaire occurs at home before students leave the house. If any of the below cannot be attested to (one statement or more), please keep your child at home. If all the below can be attested to, your child is certified to attend school.

I attest that my child:
  • Has not knowingly been in close or proximate contact in the past 14 days with anyone who has tested positive through a diagnostic test for COVID-19 or who has or had symptoms of COVID-19;
  • Has not tested positive through diagnostic test for COVID-19 in the past 14 days;
  • has not experienced any symptoms of COVID-19, including a temperature of greater than 100.0F in the past 14 days (symptoms of COVID can be found on the CDC website: https://www.cdc.gov/coronavirus/2019-ncov/symptoms-testing/symptoms.html;
  • Has not traveled internationally or from a state with widespread community transmission of COVID-19 per the New York State Travel Advisory in the past 14 days. More information regarding this Advisory and a list of designated states can be found here: https://coronavirus.health.ny.gov/covid-19-travel-advisory;
  • And, I understand it is my responsibility, in the interest of public health, to keep my child home when sick.

Please review the directions and FAQ carefully. These documents are thorough and can guide you as you seek to meet this requirement and complete this required daily questionnaire for your child(ren).
