It is only necessary to complete Part I on the Application and the entire Physical Fitness Certification form.
The Physical Fitness Certification form must be signed by the School Nurse. The nurse is available before first period, during homeroom or right after last period to review the documents. After the nurse signs the document, please bring the paperwork to the Counseling Center for Processing.
**** During the summer months and when class is not in session, you must present a signed stamped medical certification form from a physician.
* NOTE: The working card is valid for two years and may be used for an unlimited number of successive job placements. Your employer is required to return the card to you upon termination of your employment.
The state requires that the student sign the working card in the presence of the issuing individual, therefore the student must be present when picking up the working card from the high school.
